The ACP Patient Advocate is a vital member of the Koda Health care team, guiding patients and their loved ones through meaningful, longitudinal conversations as they navigate serious illness. In this role, you will work closely with complex patients in a virtual/telephonic setting to:
- Define their values and ensure those values are shared with their loved ones.
- Provide ongoing support and check-ins to ensure patients receive the care they value as their health needs evolve.
Additionally, you will screen for palliative care and hospice needs and effectively communicate these to the care team.
This fully remote position offers flexibility in scheduling and a clear path for career growth. We are looking for a detail-oriented, empathetic individual who will serve as the primary point of contact for seriously ill patients and their caregivers. You’ll leverage Koda’s proprietary technology to facilitate meaningful conversations and complete essential documentation. Alongside technical support, you’ll provide compassionate guidance through challenging decisions, helping to improve both individual patient experiences and overall communication efficiencies.
- Conduct structured, guided discussions to review care preferences and align patients and loved ones on these preferences
- Maintain strong empathy for seniors / seriously ill users as they make difficult decisions about their healthcare journey
- Provide ongoing support for patients enrolled with Koda, including follow-up visits with patients and loved ones, patient check-ins, ongoing education and documentation revisions as needed
- Collaborate with Patient Support team (including other ACP Patient Advocate and Patient Support Associates) to most effectively engage and support patients in this process
- Prepare for patient encounters by reviewing patient notes and documentation
- Effectively screen patients for additional palliative care needs that need to be communicated to primary care team
- Document and track patient encounters throughout the Koda Health communication lifecycle, from referral to completion and ongoing support
- Maintain and improve efficiency of the entire patient communication process
- Provide ongoing feedback to supervisors regarding the patient experience
- Notice even the small details and continually brainstorm solutions to improve our processes and each patient's experience
A sample of your first 30/60/90 days at Koda:
- In the first 30 days — Develop a deep understanding of Koda’s platform, processes, and mission
- In the first 60 days — Begin managing patient support relationships independently
- In the first 90 days — Identify process improvements, refine communication methods, and track metrics to demonstrate your impact
Requirements
Qualifications
- Must have active and unrestricted licensure as a Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW), meeting all state certification and licensing requirements for the state the candidate works in.
- Minimum of one (1) year of nursing or social work experience related to management of chronic or terminal conditions in a healthcare setting.
- Must be available to work Monday-Friday during regular business hours and occasional evenings.
- Knowledge of serious medical conditions (such as cancer, dementia, CHF, COPD, cirrhosis, ESRD, etc.) and their progression.
- Strong verbal communication skills, with a highly empathetic and supportive demeanor
- Ability to explain technical concepts in detail to users unfamiliar with technology
- Strong attention to detail
- Proactively seek solutions and look for opportunities to improve processes
- Proven track record of thriving in a fast-paced environment where critical thinking and proactive teamwork are essential for success
- Team player eager to expand their skill sets and grow within the company
