Hi, we're Oscar. We're hiring a Community Health Worker to join our Case Management team.

About the role
As a Community Health Worker you will support members by providing resources and education towards reaching chronic disease self-management goals.
You will report to the Case Management Manager.

If you live within commutable distance to our New York City office (in Hudson Square), our Tempe office (off the 101 at University Dr), or our Los Angeles office (in Marina Del Rey), you will be expected to come into the office at least two days each week. Otherwise, this is a remote / work-from-home role.

Pay Transparency:
The base pay for this role in the states of California, Connecticut, New Jersey, New York, and Washington is: $26.67 per hour. The base pay for this role in all other locations is: $24.00 per hour. You are also eligible for employee benefits and monthly vacation accrual at a rate of 10 days per year.
Responsibilities
- Assist members in following an outcome-based plan of care, based on the member's input and assessment results and identify/address current needs of the member, using outlined escalation pathways when needed.
- Outreach identified members to identify and help solve for barriers that may affect their care by phone outreach, secure messaging and mail.
- Monitor care and services provided to members to ensure that quality care is being delivered in the most appropriate setting
- Ensure the "Plan of Care" moves with the member and update the care plan as the member's health status changes
- Assist members by providing resources, education and information promptly.
- Compliance with all applicable laws and regulations
- Other duties as assigned
Qualifications
- High school diploma or GED
- CHW certification required from approved Community Health Certification Program, or two years of equivalent work experience
- 1+ years experience care coordination and navigation
Bonus Points
- Bilingual [English and Spanish or Creole]
- AA or Bachelor's degree in social science or health related field
- Behavioral Health Experience

Pay Transparency: Final offer amounts, within the base pay set forth above, are determined by factors including your relevant skills, education, and experience.
