Genomic Life is seeking a full-time Genomic Clinical Navigator whose expertise and training can help support and guide members to make informed decisions on their healthcare journey. The Genomic Clinical Navigator is responsible for the day-to-day management of Genomic Life members, using their genomic expertise and training to help guide members to make informed decisions while providing emotional support and advocacy throughout the care continuum. The navigator will partner with the healthcare team to facilitate timely access to genomic-based solutions, linking members to precision medicine, to deliver actionable insights along their healthcare journey.

Responsibilities:

The Genomic Clinical Navigator is positioned as a critical support figure in guiding individuals through the complexities of genomic-based healthcare.
- Offer information, education, and care coordination on various genomic services, including genetic health screening, carrier screening, pharmacogenomics, and molecular profiling.
- Facilitate a range of services from second opinion pathology reviews to genomic profiling while liaising with local healthcare groups, cancer centers of excellence, high-risk clinics, and service providers.
- Support the healthcare provider team to receive timely translated and actionable results to facilitate quality outcomes and guideline-concordant care.
- Prepare appropriate clinical cases to be presented at the Molecular Tumor Board, document discussion, and provide clinical follow-up.
- Coach members in goal identification, shared decision-making, and communication with their healthcare team.
- Provide NCCN distress and barrier assessments at key points along the care continuum to assess social determinants of health (SDOH), barriers to care, and distress to proactively mitigate and provide resources, support services, and education.
- Educate and advocate for prevention behaviors, early detection, and healthy behavior education.
- Assess population health factors and conduct risk assessments, including lifestyle factors.
- Practice with compassion, respect, and empathy for the inherent dignity, worth, and unique attributes of every person.
- Adhere to culturally sensitive boundaries and provide navigation services that are culturally and linguistically appropriate.
- Participate in tracking and monitoring metrics and outcomes, collaborating with administration, and reporting findings to relevant stakeholders.
- Advocate for patients by upholding their autonomy in decision-making, ensuring informed consent, and assisting patients in families in expressing self-determination.
- Demonstrate and maintain a working knowledge of evidence-based information in genomic navigation practice by engaging in ongoing professional development.
Requirements:
- 5+ years of clinical experience and 2+ years of navigation experience.
- Proficient in Microsoft Office and Zoom and ability to learn new software (Salesforce.com).
- Exceptional emotional intelligence and compassion.
- Demonstrate an understanding of the healthcare landscape, healthcare systems, national and community resources, and services across the healthcare continuum to support and assist patients to overcome barriers and increase access to care.
- Knowledge of the care trajectory, genomic testing, precision medicine, and NCCN guidelines.
- The ability to work independently and as part of a close-knit team.
- Strong organizational and communication skills with the ability to multitask.
- Strong critical thinker and flexible in your approach to challenges and opportunities with analytical and problem-solving skills.
- Genetic, cardiology, and/or oncology experience is a plus.
Required Education:
- Bachelor of Science Degree or Master’s degree in a relevant field.
- Has navigation certification or has completed at least 36 hours of genomic-based continuing education over the last 3 years.
Preferred Qualifications:
- 1+ year of telephone triage experience.
- Certified Genetic Counselor (CGC), Cardiovascular Nursing Certification (CV-BC), Oncology Certified Nurse (OCN), or other relevant specialty certification.
- Case management experience.
- Experience with Clinical Research/Clinical Trials.
- Experience with Genomic testing and precision medicine.
- Experience with genomic clinical research.
- Experience with health education, patient advocacy, and prevention and screening.
