TCARE includes standardized assessments, scientifically validated decision algorithms, care planning tools and community resource links. TCARE has impacted over 25,000+ caregivers thus far by over 400+ care professionals in 17 states. TCARE will continue to grow as the State of Washington has demonstrated a $10 Million annual savings, delayed placement of care recipient by 18-24 months and reduced Medicaid service use by 20%. CMS has approved TCARE to be a Medicaid reimbursable line item starting Oct 1st.
