We currently serve care providers and health insurers across the country. In the hospital, we reduce falls and delirium by 80%+, and at home, we encourage self-management of multiple chronic conditions to avoid (re)admissions to the hospital and provide psychosocial support and coaching to avoid unnecessary emergency department visits and inefficient use of home care resources.
