The typical US senior sees seven physicians across four practices plus insurers, pharmacies, family members, and facility staff. No one has the full picture. Fragmented data in incompatible systems leaves seniors and families managing chaos instead of focusing on living well.

Leland is building the first unified system of record for aging to help seniors live longer, healthier, and happier lives. With one complete record of a senior's health and life, AI can intelligently analyze the full picture to surface risks, gaps, and opportunities, while the senior's trusted human navigator guides the next best actions and removes friction so families can actually follow through. This turns fragmented information into proactive, coordinated support that extends healthy, independent years.

We have a multi-year funding runway and are launching pilots in 2026 with providers and already in active conversations with major US health systems. The company is led by a Stanford GSB founder with experience scaling new businesses at Sword Health and Homebase.

About the role

As the Founding Head of Care Delivery, you are our first navigator and the person who builds the navigation function. For the first several months you carry the patient panel yourself, and then you turn that experience into a playbook and hire the navigation team.

You will own four things:

• Deliver navigation yourself: Personally carry the first cohort of seniors and families. Enroll patients, close care gaps, support caregivers, catch red flags, and escalate. You are the front line before there is a team.
• Build the navigation operating system: Turn the work into a repeatable playbook. Define the journey, protocols, scripts, escalation rules, QA, and training so the next navigator can deliver the same quality you do.
• Hire and lead the team: Recruit, train, and manage the navigators who come after you. Build a team that holds the bar you set.
• Measure and improve: Own enrollment, engagement depth, care gap closure, and patient NPS. Use the data to make the playbook better every week.

You deliver the work and build the function that scales it. Expect to move fast and own outcomes end-to-end with minimal process.

Who we're looking for

• You have personally helped people navigate healthcare, eldercare, or a similarly high-stakes, high-friction system, and you are exceptional at it.
• High agency and deep ownership. You figure things out, move fast, and do whatever it takes to get a senior the outcome they need.
• You thrive in ambiguity. You prefer clear goals and maximum freedom over hand-holding and process.
• A clinical or social work license (RN, LCSW, SW) is a plus, not a requirement. We hire on advocacy talent and owner mindset first.
• Low ego, high output. You want the intensity and ownership that only exists at the earliest stage of a company.

If you need constant direction, clear roadmaps, or a well-defined role, this is not the right place.

Compensation and benefits

$110k-$130k salary + generous founder shares. We also provide healthcare benefits.
