Leland Care is building the first unified system of record for aging.
The typical US senior sees seven physicians across four practices, plus insurers, pharmacies, family, and facility staff. No one holds the full picture, and that fragmentation leaves seniors and families managing chaos instead of focusing on living well.
We pull a senior's entire health and life into one complete record, then make it actionable in two ways: AI that continuously analyzes the full picture to surface risks, gaps, and opportunities, and a trusted human navigator who guides the next best actions and removes the friction so families can actually follow through. Fragmented information becomes proactive, coordinated support that extends healthy, independent years.
We're a pre-seed-funded, in-person team in New York, launching provider pilots in 2026 and already in conversations with major US health systems. We're led by a Stanford GSB founder who scaled new businesses at Sword Health and Homebase.
We're hiring our founding team: engineers, operators, and care leaders who want extreme ownership, move fast, and want to fix a broken part of healthcare for the people who need it most.
