QC is developing a software platform for mobile patient monitoring, management, and engagement to allow healthcare organizations to better manage patient health even when those patients are not in the clinical system. The platform is composed of two main software tools: a patient mobile app and a clinical management portal.

Patients complete regular health assessments, which capture information including symptoms, drug adherence, and data from connected medical devices. This information generates personalized feedback to help patients become educative and engaged participants in managing their health. QC’s clients utilize the management portal with existing clinical assets such as case management nurses. From the management portal, nurses monitor patients through real-time access to patient-captured information and alerts.

QC’s platform also keeps doctors, nurses, family, friends, and any other caregivers up-to-date with the health of their patient or loved one.
