As a result, they struggle to deliver results in sustained behavior change, improved patient health outcomes, admissions avoidance, and obesity-related care cost reduction. ProjectVision takes a big data approach using the most ubiquitous technology device across all demographic groups - the smartphone - as our method of data collection. We sell a platform to Managed Care Organizations to help them better engage Medicaid patients with personalized chronic condition management support via a smartphone application. We identify psychometric segments within a population, stratifying based on psychological readiness for lifestyle change. This helps providers offer a culturally relevant funnel of services that help patients achieve successful self-management, no matter their current readiness for change. Our approach helps providers overcome some of the key factors that prevent patient engagement in the context of chronic condition management, and helps them realize ROI in the form of improved health outcomes and reduced cost burden of obesity.
