APluscare provides care and support services to individuals with various health care needs in the areas of personal care, behavioral support, recreational opportunities and employment services. APluscare is dedicated to providing person-centered, comprehensive quality care and support services to individuals and families in their place of residence, including supervised apartments. APluscare focuses on each individuals’s complete and unique health care needs to provide a wrap-around care/support system. This includes, but is not limited to, their safety/environment and social support system. APluscare is committed to providing behavioral health services that assist each individual to maximize their well-being in a manner that makes sense to that person. APluscare is committed to allowing individuals to participate in health care choices and decisions, respecting their rights and needs for self-determination. APluscare provides treatment and support services to individuals without regard to age, race, religion, sex, nationality or disability. On an ongoing basis, APluscare provides home/behavioral health care education to individuals and their families, staff members, and the community. We monitor and evaluate performance outcomes/quality improvement utilizing a periodic service review.
