Currently, most social determinants of health and mental health concerns go unaddressed and physicians are not equipped with the tools needed to intervene and address them. The Vertex 360 Platform would provide physicians with a trauma-informed community referral platform they could use to provide trauma-informed, patient-centered referrals and care. There are three main aspects to the platform. First, a psychosocial assessment tool to assess social determinants, impacts of trauma, and other mental health risk factors. Second, a trauma-informed care plan that includes a questionnaire to promote sensitive, patient-centered care during the visit. Finally, a psychosocial resource platform that consists of a network of evidence-based accessible psychosocial resources within the patients community. The current minimum viable product of this web-based community referral platform has been completed and can be used as a crude reference for the vision/goals of the platform.
