Physicians in Canada spend hours every week on insurance and government forms: disability statements, the Disability Tax Credit, provincial benefit applications, return-to-work and life-insurance reports. Most of the answers are already in the patient’s chart. The work is finding them, copying them accurately and getting the form signed, while patients wait for income support they need.

Prometheus Codex does that groundwork. A clinic uploads the chart documents and picks the form. Codex reads the documents, and the patient can answer a short questionnaire from home. Codex then drafts each factual field on the form, and every drafted value cites the exact passage it came from. The physician reviews the draft on one screen, edits anything, and signs. Codex never writes a diagnosis, a prognosis, a functional assessment or an opinion on the physician’s behalf. Those stay with the clinician, by design.

The hard problems are the interesting ones. We extract facts reliably from messy real-world documents. We know when not to answer: a field the chart can’t support is left for the physician, not guessed. We onboard hundreds of forms as data rather than code, and we protect patient data, which stays in Canada. Every clinician action is audited.

We’re physician-founded and based in Kelowna, BC. Our founder is a practising physician who signs these forms every week, and who built the platform. Codex is live in pilot with BC family practices. We’re growing our form library province by province.
