The Klio CarePlan product is a secure, web and mobile-enabled system that, based on a patient’s medical history and demographics, generates personalized care plans by applying analytics to structured and unstructured data. A key component of each care plan is a scalable framework for collecting and processing patient-generated health data in the form of patient self-assessments as well as data streams from third-party apps (e.g., diet tracking tools) , wearable devices (e.g., digital scales) and sensors (e.g., accelerometer). By contextualizing the patient-generated data against the patient’s individual care plan, we provide automated feedback to both patients and providers such that the data becomes relevant and actionable. In this way, Klio enables healthcare providers and their patients to truly benefit from the increasing availability of patient generated health data in not only engaging patients but being better able to manage risk.
