Gather Health helps people with chronic diseases receive continuous care from their doctors and support from their family. 

An individual views their treatment plan on a smartphone app, and records whether or not they completed each task. Doctors and other healthcare providers view this data through a web portal to monitor their patients' progress, provide advice and encouragement, and adjust treatment plans as needed. A unique feature of Gather Health is a companion app for family members of people with chronic diseases, which educates them on their loved one's disease and facilitate meaningful engagement and support.

Gather Health provides patients ongoing attention and feedback between regularly scheduled visits in order to better understand their disease and manage it more effectively. In addition, by providing a channel for remote care, the system creates revenue for the doctors, reduces costs for insurers, and generates valuable data.
