Your medical history is scattered across every hospital, clinic, and lab you have ever visited. Getting it means a different portal for each one, faxes, fees, and weeks of waiting. And when the records finally arrive, they are thousands of pages most people cannot read.

ClearChartAI fixes both halves of that problem. A patient verifies their identity once, and we retrieve their available records from across the country through the national health information networks, often in minutes. We organize everything into one timeline and explain it in plain language, with every answer citing the page it came from. The patient owns the record and decides who sees it, from family members to their doctors.

We are a certified Individual Access Services provider under TEFCA, connected through CommonWell, and one of five apps listed there for patient access. We are live in production with Epic and have retrieved more than 1,500 records. Clari, our AI assistant, answers patients' questions about their own chart by text or voice, grounded in their records and never replacing their care team.

ClearChartAI was founded by Nicholas Davis, a cardiology nurse practitioner who spent years in ICUs watching care slow down because no one had the full history. Our founding engineer, Vishnu Koraganji, leads the platform, and Dr. Basil Alkhatib, Chief of Cardiology at Banner University Medical Center Phoenix, chairs our clinical governance. We are backed by angel investors and are part of the C10 Labs AI Nexus accelerator in New York.

We are small, clinician-led, and move fast. You will work directly with the founders, see your work go live within days, and help build something patients actually need. We care about doing it right: verified identity, patient consent, and honesty about what the record does and does not say.
