Amplify Health is helping primary care physicians (PCP) reassert their role as the principal navigator of their patients' healthcare journeys. Our software enables PCPs to direct contract with self insured employers (SIEs) and proactively manage their patients. When doctors are being paid for outcomes, they have one mandate: create effective treatment plans that their patients follow.

We created the software to measure and achieve this mandate. We combine medical record data (the plan of what should happen) with claims and other sources (what's actually happening) and surface two major deviations that are indicative of future avoidable healthcare costs.

Off the Plan: not adhering to prescribed treatment plan e.g. did not take $50 medication
Off the Ranch(OTR): utilizing healthcare without the PCPs knowledge e.g. $50,000 self referral to spine surgeon
The examples in the 3rd slide are outputs from our rules engine that identified OTR activity
