The Activcore Continuum Health Network provides integrated Physical Rehabilitation, Medical Fitness, Nutritional, and Population Health Management services.

Furthermore, the Activcore digital health platform is not only a SAAS-based “A to Z” practice management platform for the provider, but a Health Information Exchange built to communicate data between our network and various health IT systems using Healthcare Interoperability Standards. Activcore’s core competency becomes collecting objective and subjective data to provide empirical proof of improved health outcomes within a true coordinated-care model.
