Founding AI & Backend Engineer

Vitarc is building the AI layer for out-of-hospital care. We're starting with chronic disease management in Saudi Arabia: a market with one of the world's highest diabetes and hypertension burdens and a national push to modernize healthcare. Our product carries the load patients currently carry themselves: it tracks their state, follows up on care plans, coordinates medications and appointments, and surfaces the right next step at the right time.

This is the hardest and most important engineering role at Vitarc, and we're looking for someone genuinely exceptional to own it. The backend isn't a CRUD app behind a mobile client. It maintains a live model of each patient's state, decides what should happen next, and drives the workflows that make the app feel proactive instead of reactive.

You'll own the backend, the AI layer, and the orchestration between them. You'll work directly with the founders, make the core architectural decisions, and have the kind of end to end ownership that's only possible this early.

What you'll own

• Backend architecture, APIs, data models, and cloud infrastructure, designed from the ground up
• The orchestration engine: a stateful system that tracks patient state and triggers the right care workflows, reminders, follow-ups, and next-step actions at the right time
• The AI layer: agentic, multi-step workflows, summarization of clinical and patient data, and intelligent task coordination
• A fine-tuning and evaluation pipeline for models trained on clinical/patient data, including the data generation, guardrails, and eval harnesses that keep outputs safe and reliable
• Event-driven pipelines for documents (OCR / parsing), symptoms, medications, appointments, and vitals
• Scheduling logic and patient-specific triggers that are correct, observable, and reliable
• Data handling that respects healthcare-grade privacy and Saudi data-residency requirements (PDPL)

What we're looking for

• Exceptional backend engineering: APIs, databases, auth, queues, and cloud infrastructure at production scale and reliability
• Real experience building AI products beyond simple chat: agents, tool use, retrieval, and stateful multi-step workflows
• Hands-on experience with LLM orchestration and, ideally, fine-tuning and evaluation. Not just calling a chat API
• The ability to think in systems, state machines, and workflows rather than screens
• Strong product judgment and the independence to design something hard from scratch and ship it
• A high bar for correctness and reliability and honesty. This is healthcare, and a missed or wrong action has real cost

• Event-driven / distributed systems and workflow engines (e.g., Temporal)
• Agentic frameworks and stateful orchestration (e.g., LangGraph, or your own)
• OCR and document-parsing pipelines
• Vector search / RAG over private data
• Healthtech, medtech, or other regulated products
• Healthcare data models (FHIR) or Saudi health infrastructure (NPHIES)
• You've been a founding or early engineer and have taken something from zero to production
