About the job

How It All Started

In 2025, we started Pythagoras AI around a simple belief: healthcare cannot keep scaling by adding more people. There are already major labor shortages across the industry, while patients need more outreach, follow-up, coordination, and support than healthcare organizations can realistically provide with human teams alone.

That is why we are building Clara, an AI agent workforce for healthcare. Clara works alongside healthcare organizations to take on patient-facing work across scheduling, care management, screenings, patient outreach, inbound support, post-discharge follow-up, and other operational and clinical workflows. We integrate deeply into our customers' EHRs, telephony, and existing processes so Clara can perform real work, not just answer questions.

Our long-term vision is much bigger than automating calls. As Clara learns how each healthcare organization operates and builds longitudinal context about its patients, she can become increasingly proactive and autonomous. We believe AI can dramatically expand the capacity of healthcare, allowing organizations to serve more patients, engage them more frequently, and move from reactive care toward continuous, proactive care.

The Team

We are a team of engineers, clinicians, and entrepreneurs building at the intersection of AI and healthcare. Our founders bring experience across AI, healthcare, and enterprise technology, and we are building a team that combines exceptional technical ability with a deep understanding of the responsibility that comes with working in healthcare.

We move fast, go deep into our customers' workflows, and take ownership of outcomes. Healthcare is complex, and we do not believe the future will be built by generic AI layered on top of existing systems. We believe the winning products will deeply understand how healthcare organizations operate and reliably perform the work alongside their human teams.

What we are building has the potential to do more than reduce administrative burden. By helping solve healthcare's labor shortage, we can make high-quality care accessible to far more people and give doctors, nurses, and care teams more time to focus where human judgment, skill, and empathy matter most.

Mission Statement

Build the AI agent workforce for healthcare, expanding the capacity of healthcare organizations so high-quality, proactive care can reach everyone who needs it.

Our Values

  • Patients first, always. Safety, trust, and dignity come before everything.
  • Quality and safety above all. Reliability and precision matter when people's health is involved.
  • Own the outcome. We do not stop at building technology. We make sure it works in the real world.
  • Go deep. Healthcare is complex. We learn our customers' workflows, constraints, and operating logic in detail.
  • Move with urgency. The problems we are solving are important, and the technology is moving quickly.
  • Stay curious. We question assumptions, learn continuously, and keep pushing what AI can do.
  • Build together. High ownership, low ego, and shared purpose define how we work.

The Role

We're hiring our first Forward Deployed Engineer to sit between our customers, our sales lead, and our engineering team. You'll own the customer-facing side of product development: understanding what healthcare providers actually need, shaping how Clara behaves for each customer, and shipping the changes that make Clara work in the real world.

This is not a support role and not a pure engineering role. You'll talk to customers daily, write code when needed, and have direct influence over what we build next.

What You'll Do

  • Own customer relationships post-sale. Be the primary technical point of contact for live accounts. Run regular check-ins, troubleshoot issues, and make sure Clara is delivering value.
  • Translate customer needs into product. Work with our sales lead to understand what new and prospective customers want. Identify which requests are one-off configuration vs. real product gaps. Bring that signal back to engineering.
  • Ship customer-specific changes. Clara's architecture is designed for per-customer customization through strategy files. You'll write and modify these strategies, tune prompts, adjust call flows, and ship small features end-to-end.
  • Drive product improvement. You'll have more direct customer exposure than anyone else on the team. We expect you to form strong opinions about what to build next and advocate for them.
  • Support implementation. Help onboard new customers — EHR integration discovery, use case scoping, call flow design, go-live coordination.
  • Improve how we operate. Build internal tooling, write runbooks, identify recurring issues, and make the team more effective over time.

What We're Looking For

Required:

  • 5+ years in a role that combined engineering and customer-facing work (FDE, solutions engineer, implementation engineer, technical CSM, or similar)
  • Comfortable writing and reading TypeScript/Node.js. You don't need to be a senior IC, but you should be able to ship a feature, debug a bug, and review a PR
  • Strong written and verbal communication. You'll be on calls with clinical operations leaders, compliance teams, and engineers, sometimes in the same week
  • Track record of identifying product gaps from customer conversations and driving them to resolution
  • Self-directed. You'll often be the only person at Pythagoras in a customer conversation

Preferred:

  • Healthcare experience — you've worked with clinical, operational, or compliance teams at a healthcare company. Comfortable with HIPAA, EHR workflows, and conversations with clinical operations leaders.
  • Experience with voice AI, conversational AI, or LLM applications
  • Familiarity with AWS, GraphQL, React etc
  • Experience at an early-stage startup (Series A or earlier)

Compensation & Logistics

  • Competitive base salary + meaningful equity
  • Fully remote within the US
  • Health, dental, vision
  • Travel to customer sites occasionally (estimated <20%)
