Essential Duties:
Assess, plan, implement, and evaluate patient care needs to enhance care quality and improve patient satisfaction.
Ensure seamless care coordination and cost-efficiency through case management, utilization review, and discharge planning strategies.
Supervise care coordination and discharge planning for all patients to ensure a smooth transition throughout their care journey.
Set departmental goals and performance standards to uphold excellence in case management services.
Integrate social services and case management functions into patient care and discharge planning to ensure comprehensive support.
Conduct real-time medical record reviews using established guidelines and criteria to maintain care quality and regulatory compliance.
Serve as an advocate for patients by investigating adverse events and providing necessary staff training and support.
Leverage resources and engage with patients to achieve clinical goals and optimal outcomes.
Ensure appropriate use of services from admission through discharge to maximize care efficiency and reduce unnecessary costs.
Monitor patient satisfaction and the overall quality of care provided, ensuring continuous improvement.
Foster strong communication and collaboration with physicians and the healthcare team to improve patient outcomes.
Assist physicians in managing costs while ensuring high-quality care and patient outcomes.
Guide patients and families through the clinical case management process, explaining roles, responsibilities, and available resources.
Facilitate interdisciplinary rounds and care conferences to ensure well-coordinated and holistic patient care.
Work with clinical teams to develop, implement, and adjust patient care plans based on individual needs and goals.
Develop and update patient care protocols and policies to ensure they reflect current best practices and maintain high standards of care.
Coordinate and deliver social services to patients and families, ensuring they receive the necessary emotional and practical support throughout their care.
Necessary Skills:
Education: Must be a graduate of an accredited nursing program with a Bachelor of Science in Nursing (BSN) or higher.
Certifications: Basic Life Support (BLS) certification required upon hire.
Licensure: Current Arizona Registered Nurse (RN) license or Compact State RN license required.
Experience: At least 2 years of experience as a Director of Case Management in a critical care hospital setting.
Recommended Certifications:
Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification preferred.
Preferred Education: Master’s degree in a relevant field preferred.
Leadership Experience: A minimum of 5 years in case management leadership or senior leadership roles in critical care hospital management preferred.
